• Identification of the material that was removed and the location where it appeared before removal

• A statement under penalty of perjury that you have a good-faith belief that the material was removed or disabled as a result of mistake or misidentification

• A statement that you consent to the jurisdiction of the appropriate court in your area, or, if you are located outside the United States, to the jurisdiction of an appropriate court where HomeVienne is located

• A statement that you will accept service of process from the party who submitted the original DMCA notification
